The responses you provided in the eligibility survey (called the medical history survey) will be used for research.
You will be supplied with an at-home A1C test kit. We will send a test kit containing two A1C tests (one each for the beginning and end of the study) to complete. Your A1C test kit will come with instructions on how to collect and return the sample.
Participation in the study will require you to agree to Unicity’s Study Participant Waiver and Study Informed Consent. Please review these documents carefully.
You must remain subscribed to Feel Great for the duration of the 90-day study and complete the study requirements to receive the product credit.
Study participants will be required to use the Feel Great Fasting app to track daily study participation such as recording meal times and product usage. Optional: If you have access to a scale and tape measure, you may add additional metrics such as your weight and waist circumference.
We will ask you to complete a short survey at the end of the 90-day study.
You may choose to voluntarily withdraw from the study at any time.
If you or the Unicity study team withdraws you from the study, you will receive Unicity product credit for successfully completed milestones, but not product credit for subsequent study milestones. You may continue to use the Feel Great Fasting app and products.
If you have been diagnosed with type 2 diabetes or are on glucose control medications, please consult with your doctor before beginning this study.
Unicity will use the data and self-reported information collected through the app, A1C test, and final survey.
